Nursing Support Services

Rationale
Our Nursing Support Services provides a registered nurse, licensed in the State of Maryland, to perform Nursing Consulation, Health Case Management, and Delegation services, based on the participants assessed need to ensure they are safe and healthy during service delivery.
Scope
Nursing Support Services include:
A. Performance of an initial nursing assessment to include reviewing and identifying health needs, review of medical care and appointments, review of all physician orders, and identifying nursing needs to determine:
a. whether the program participant requires continued or additional nursing tasks;
b. the participants health needs require performance of nursing tasks, including administration of medication
c. if tasks are delegable in accordance with the MBON regulations and
d. if the participants nursing tasks are exempt from delegation in accordance with the MBON regulations.
B. Based on the initial nursing assessment, if the participant requires, and meets criteria to receive, Nursing Consultation services, then the registered nursing providing Nurse Consultation services must:
a. Provide recommendations to the participant on how to have their health needs met in the community, including accessing health services available in the community and other community resources;
b. Development or review of health care and emergency protocols for the participant, uncompensated caregivers for use in training the DSP; and
c. Development or review of communication systems the participant may need to communicate effectively with health care providers, DSP’s, and uncompendated caregivers who work to ensure the health and safety of participants; and resources in the community that may be needed to support the participants health needs.
C. Based on the intial nursing assessment, if the participant requires, and meets criteria to receive, Health Case Management Services, the registered nurse providing Health Case Management services must:
a. Provide recocommendations to the participant on how to have their health needs met in the community, including accessing health services available in the community and other community resources;
b. Develop a nurse Care Plan and protocols regarding the particpants health needs; and
c. Provide training to DSP’s on how to address the participants specific health needs, in accordance with the health care plans and protocols developed.
D. If during the initial nursing assessment it is determined that the program participant requires Degation services, then the registered nurse will:
a. Provide recocommendations to the participant on how to have their health needs met in the community, including accessing health services available in the community and other community resources;
b. Develop a nurse Care Plan and protocols regarding the particpants health needs;
c. Provide training to DSP’s on how to address the participants specifc health needs, in accourance with the health care plans and protocols developed;
d. Monitor the DSP/CMT’s performan of delegated nursing tasks, including reviewing applicable documentation in accorance with applicable regulations and standars of nursing care;
e. Complete a Nursing Assessments and review health data documented and reported by DSP, in accordance with applicable regulations and standards of nursing care; and
f. Provide on-call service to DSP’s who are performing delegated nursing tasks, while delegation is occurring, for up to 24 hours per day, 365 days per year to address participants health in cases of emergency.
E. Review of the Health Risk Screening Tool (HRST) at Level 3 or above, on an annual basis and when any significant changes in the health of the participant occur;
F. Completion of the Medication Administration Screening Tool, on an annual basis and when any significant changes in the health occurs, to determine the level of support needed for medication administration.
Staffing and Training
Staff and volunteers working for or contracted with Virtuous who provide supports services or spend any time alone with a participate will meet the following requirements:
• possess a valid Maryalnd and/or Compact Registered Nursing license;
• successful completiong of the DDA RN Case Manager/Delegating Nurse (CM/DN) Orientation training within 90 days of first providing services;
• upon completion of DDA’s training, maintain active status of DDA’s registry of DD RN CM/DNs;
• be active on the DDA RN CM/DNs;
• complete the online HRST Rater and Reviewer training;
• attend mandatory DDA trainings;
• attend all DDA provider nurse meetings;
• pass a criminal background investigation and any other required background checks and credentials verification as providd in Appendix C-2a;
• First-Aid and CPR Certified;
• pass a criminal background check via CJIS;
• participate in in-service training on the Person-Centered Plan;
• complete orientation and training as designated by DDA;
• possess a valid drivers license if transporting participants; and
• have automobile insurance for all vehicles that are owned, leased, and/or hired and used in the provision of services.
Setting and Location
Services will be provided in the Central Regions of Maryland.
