Our Mission

Our mission is to provide individualized supports to adults with intellectual disabilities to ensure they can realize personal goals, live in communities that foster independence, and participate in rewarding and meaningful experiences within integrated communities and settings.

About

Model of Care

Our model of care is composed of progressive independence in an environment that provides ongoing support based on individual needs and desires.  Our goal is to deliver quality services while providing useful resources to assist people in becoming as self-sufficient as possible while accomplishing their individual goals with the desired outcomes. 

Our model of service is based on the five essential focus areas for community living: 

  • Supporting Families
  • Self-Advocacy
  • Self-Determination
  • Employment
  • Independent Supported Living

Programs and Services

Transportation

Transportation services will be designed to improve the program participant and family caregiver’s ability to access community activities where the participant lives, works, shops, or regularly spend their time.

Supported Living

Our Supported Living services provides assistance to individuals who reside in their owned or leased personal properties in a community-based setting. Services are geared to support the individual in becoming more self-sufficient in their homes and communities.

Respite

Under our Respite Services model, we provide families and people with short-term relieve services provided when a regular caregiver is absent or needs a break.

Personal Supports

Our Personal Support Services module assists participants with personalized services to promote independence, build and maintain skills, and community integration in their homes and communities.

Nursing Support Services

Our Nursing Support Services provides a registered nurse, licensed in the State of Maryland, to perform Nursing Consulation, Health Case Management, and Delegation services, based on the participants assessed need to ensure they are safe and healthy during service delivery.

Employment Services

Our Community Living/Group Home Services provides opportunities for participants for sharing of lives, forming of caring households, and therapeutic relationships in setting where they are treated with dignity and respect. Our goal is to provide skills to develop, maintain and acquire skills to assist with daily living skills, socialization, and advocacy in a community residential setting.

Our Community Living/Group Home Services provides opportunities for participants for sharing of lives, forming of caring households, and therapeutic relationships in setting where they are treated with dignity and respect. Our goal is to provide skills to develop, maintain and acquire skills to assist with daily living skills, socialization, and advocacy in a community residential setting.

Community Living

Our Community Living/Group Home program provides long-term sharing of lives, forming of caring households, and close relationships between people with disabilities and qualified support staff.  Services include but are not limited to:
